GHD’s Community Coaches work to maximize the success of the eleven communities funded by Healthcare Georgia
Foundation for The Two Georgias Initiative. This is a consulting position to assist three communities in Central and South
Georgia in the implementation of their Community Health Improvement Plans to achieve health equity in their rural

The Community Coach will:

  • Serve as a thought partner for CHIP implementation staff and board/committee members for three The Two
    Georgia Initiative communities
  • Maintain regular contact with the Project Director of the assigned communities, to include regular scheduled calls,
    ad hoc calls, site visits, etc.
  • Assist in building robust collaborations to reduce rural health disparities and improve equity outcomes
  • Facilitate development of a strategic mindset for long-term sustainability and impact
  • Assist in identification and adaptation of best practice models for community improvement
  • Facilitate peer sharing and learning among The Two Georgias Initiative communities
  • Identify the need for external expertise and broker the use of technical assistance consultants
  • Actively engage and communicate with GHD staff and HGF Management team members
  • Participate in on-going training to enhance technical, interpersonal, and cognitive skills
  • Participate in GHD team meetings and The Two Georgias Initiative Management Team Meetings



  • Bachelor’s degree in business, public health, communications, or equivalent/relevant area
  • Advanced degree preferred


  • 10 years’ experience in rural community outreach with previous work in meaningful engagement of community
    members with “lived experience”
  • Broad understanding of the opportunities and challenges of rural health and health care
  • Understanding of the issues related to social determinants of health
  • Understanding of the concepts and principles of health equity
  • Understanding of population health, health care financing, and the health care infrastructure
  • Ability to coordinate perspectives and contributions of diverse entities
  • Strong organizational skills and written and verbal communication
  • Positive public presence
  • Fluency in Spanish a plus but not require

Position Specifics:

  • Community Coach is a contracted consulting position, requiring up to two days per week
  • GHD operates as a virtual office; consultant must provide personal work space, equipment, phone, and vehicle
  • Consulting rate is dependent on experience; mileage is reimbursed at the Federal rate
  • At least quarterly travel to assigned communities is necessary, as well as attendance at GHD staff and HGF Team meetings in Atlanta on a bi-monthly schedule

The consultant will support communities in South and Central Georgia in implementation of their Community Health Improvement Plans.

Since 2016, GHD has partnered with the Healthcare Georgia Foundation to provide Community Coaches as strategic thought partners to 11 rural Community Health Partnerships funded through The Two Georgias Initiative.
